New GUIDE program offers personalized support for people with dementia and their caregivers

Senior Resource Alliance and Healthspan Partners launch tailored care plans and community resources to ease the challenges of dementia

The Senior Resource Alliance (SRA) – in collaboration with Healthspan Partners of SWFL – recently launched GUIDE, a program that benefits people with dementia.

Participants receive an individualized care plan that’s tailored to their evolving needs and have access to a Care Navigator who can connect them to community-based  resources such as food and transportation assistance. Caregivers are welcomed into an  expansive network that offers respite services, support groups, a 24/7 call line and workshops for time and stress management. 

“Dementia can be both physically and emotionally draining for everyone involved,” said  Karla Radka, President & CEO of SRA. “With close to 10 million new cases every year,  it’s more important than ever to give patients and their families a support system that  they can depend on.” 

GUIDE is open to those who meet the following criteria:  

  • People diagnosed with dementia 
  • Traditional Medicare beneficiaries, excluding Medicare Advantage and hospice • Residents living outside of long-term nursing facilities 

“No one should face dementia alone,” said Dr. Adam Perry, Founder of Healthspan  Partners of SWFL. “Each person and family affected by dementia has a unique experience. As the central hub of the local senior services network, Senior Resource  Alliance is expertly equipped to guide clients and caregivers with personalized support.” 

This partnership builds upon an eight-year program funded by the Centers for Medicare & Medicaid Services that has assisted people nationwide.
