FL-Dept of Health in Orange County shares how to replace a damaged or lost COVID-19 vaccination card

The Florida Department of Health (FL-DOH) in Orange County wants to let Orange County residents know if they damaged or lost their COVID-19 CDC Vaccination card they can request a replacement from the Department.

In order to request a replacement card, residents can visit the Department’s website at: http://orange.floridahealth.gov/about-us/expanded-alerts.html. Once there, click on the “Vaccination Replacement Card” drop-down menu. This replacement card offer is ONLY for Orange County, Florida residents.

For those who live in another Florida county, they must seek a replacement card from the Department of Health in the county they reside. If the individual received their COVID-19 Vaccination from the Veterans Administration (VA), they must get their replacement card from the VA.

The FL-DOH in Orange County reported on Wednesday, June 9, 2021 that 54.14 percent of all Orange County residents who are 12+ have received at least one dose of the COVID-19 vaccine, and the 14-day rolling positivity rate is 4.1 percent. It has been 17 consecutive day that the County’s positivity rate has been below 5 percent.

On Saturday June 5, 2021, the FL-DOH in Orange County measured the County’s 14-day rolling positivity rate below 5 percent for 14 consecutive days, prompting Mayor Demings to lift all local facemask recommendations.

However, residents may still be required to wear a facemask in locations where federal guidelines are observed (courthouse facilities, public transportation, airports), or inside of a private business.

Orange County continues to urge residents to follow the Centers for Disease Control and Prevention guidelines in order to keep the County’s positive rate low.
